POSITION: CLIENT CARE COORDINATOR As a thriving, award-winning Home Care Agency, we are seeking an energetic and team-spirited person to join our team to ensure our clients receive excellent care. This is a Full or Part Time office position that may include some field work. *POSITION OVERVIEW -You MUST have some knowledge of computers and scheduling software. -You MUST be a team player who collaborates with other office team members, including schedulers and sees the big picture. -You MUST have excellent interpersonal skills and the desire to accept the empowerment to act as the face of the company. -Bilingual (English and Vietnamese) preferred. *RESPONSIBILITIES-Customer Service and relationship management ensuring that clients receive excellent care. -Orchestrating, monitoring, and communicating care coordination details to all team members, and family members. -Identifying and addressing scheduling issues (i.e., call offs, no shows, etc) and implement solutions. -Analyzing, evaluating, and forecasting staffing level needs. -Making recommendations to owners to maintain staffing levels. -Must be open to working on-call on some weekends or as business needs require -Oversee the daily operations of the office.
